Participation Agreements
Conscious Cannabis Event Participation Agreement
This participation agreements outlines requirements to participate in group conscious cannabis experiences as well as individual, cannabis-assisted psychedelic therapy and Cannabis-Assisted psychotherapy.
Community Breathwork Participation Agreement
This is a standard transformational breathwork disclosure statement. It is signed once before attending your first Community Breathwork, then only once every calendar

Private Psychedelic Therapy & Psychotherapy Sessions
Psychedelic Therapy & Psychotherapy Practitioner Disclosure Forms are listed below and are required before your first appointment. Private sessions may also require the CCE, Breathwork or Cannabis Yoga participation agreements to cover any cannabis, breathwork or yoga related practices offered in your session.

Training Program & Safe Community Policy
The Training Registration Agreement and Safe Community Policy are required to participate in any Medicinal Mindfulness training workshop or retreat.
